    •  HIV, NRTIs

    Nucleoside reverse transcriptase inhibitor (NRTI)
    Used for treatment of HIV infection in combination with other antiretroviral agents
    Associated with higher incidence of lactic acidosis with hepatic steatosis than other NRTIs

    Indicated for the treatment of human immunodeficiency virus (HIV) infection in combination with other antiretroviral agents.

    Hypersensitivity

    Coadministration with didanosine

    • Headache
    • Chills/fever
    • Malaise
    • Insomnia
    • Anxiety
    • Depression
    • Pain
    • Rash
    • Nausea
    • Vomiting
    • Diarrhea
    • Pancreatitis
    • Abdominal pain
    • Peripheral neuropathy

    Risk of potentially fatal lactic acidosis & severe hepatomegaly with steatosis, with use alone & especially with didanosine

    Risk of potentially fatal pancreatitis when used with didanosine

    Decrease dose in renal impairment

    Severe motor weakness reported

    Risk of immune reconstitution syndrome if used in combination w/ other antiretroviral drugs

    Randomized controlled trials found lipoatrophy or lipodystrophy developed in a higher proportion of patients treated with stavudine compared to other nucleosides (tenofovir or abacavir)

    There is a pregnancy exposure registry that monitors pregnancy outcomes in individuals exposed to drug during pregnancy

    The Centers for Disease Control and Prevention recommend that HIV-infected mothers not breastfeed their infants to avoid risking postnatal transmission of HIV; based on limited data, the drug has been detected in human milk; no data are available regarding effects of stavudine on breastfed infant, or effects on milk production

    Adults

    Greater than to equal to 60 kg: 80 mg/day PO.
    Less than 60 kg: 60 mg/day PO.

    Geriatric

    Greater than or equal to 60 kg: 80 mg/day PO.
    Less than 60 kg: 60 mg/day PO.

    Adolescents

    Greater than or equal to 60 kg: 80 mg/day PO.
    30 kg to less than 60 kg: 60 mg/day PO.
    Less than 30 kg: 2 mg/kg/day PO.

    Children

    Greater than or equal to 60 kg: 80 mg/day PO.
    30 kg to less than 60 kg: 60 mg/day PO.
    Less than 30 kg: 2 mg/kg/day PO.

    Infants

    2 mg/kg/day PO oral solution.

    Neonates

    Greater than or equal to 14 days: 2 mg/kg/day PO oral solution.
    Less than 14 days: 1 mg/kg/day PO oral solution.
